Passed away on Thursday, October 29th, 2015 at the age of 52.
Loving Dad of John, Rick (Elain) and Michael.
He was happily married to Margaret for 21 years and a friend for 8 years.
Dear son of Agnes and the late John. Brother of George (Carol). Proud Papa of Gavin.
Jack will be forever remembered by his family and friends.
Memorial Service will be held in the Chapel on Thursday, November 5th at 3:00 p.m. with visitation one hour prior.
Memorial donations to the Heart and Stroke Foundation would be appreciated.
